What is the Geriatric Assessment Clinic?
As we age, our medical problems often grow more complex. Social challenges, such as not having a ride to the doctor, may interfere with good health. The Geriatric Assessment Clinic provides a thorough evaluation of an older adults' health needs. The Senior Resource Alliance, through a grant from the Community Health Improvement Council, provides the licensed clinical social worker that is part of the geriatric assessment team.

Who is on the Geriatric Assessment Team?

  • Board-certified geriatric physician
  • Geriatric fellows
  • Family practice residents
  • Licensed Clinical Social Worker
  • Pharmacist
  • Nurse or medical assistant
  • Medical and pharmacy students

Who Should Go to the Clinic?
The Geriatric Assessment Clinic is for older adults with complex medical and/or social needs. Someone may visit the clinic when experiencing:

  • Falls-gait or balance problems
  • Bladder or bowel problems-incontinence, constipation, etc.
  • Memory problems
  • Mood problems-depression or anxiety
  • Complications from taking too many medications

What do I need to bring?
To get the most out of your visit, bring:

  • Your caregiver or a family member
  • All of your medications in their original bottles. This includes:
    • Prescriptions
    • Over-the-counter medications
    • Vitamins
    • Herbal remedies.
  • The name, address, phone number and fax number of your primary-care physician
  • Records from your other physicians. This includes:
    • Lab work
    • X-ray reports
    • Other test results
    • Summaries of specialist office visits

What Should I Expect?
Your appointment will include a thorough assessment and exam. We will evaluate your:

  • Functional status
  • Gait and mobility
  • Bladder and bowel habits
  • Vision
  • Hearing
  • Memory
  • Mood
  • Medications
  • Social needs

How Much Does It Cost?
The Geriatric Assessment Clinic is part of the Florida Hospital Family Health Center, which accepts the following payment types:

  • Medicare
  • Medicaid and/or Medipass
  • Commercial insurance (HMO/PPO), if the physician is an approved provider for your plan
  • Self-payment-cash, checks and credit cards

Frequently asked questions
Q: What if I do not have a primary-care physician?
A: The assessment team can make a referral to a primary-care physician for you.

Q: Can I come to the clinic even if I do not have any of my past medical records?
A: Having records is very helpful. However, you may still make an appointment if you do not have any records.

Q: Can I come to the clinic even if I do not have a family member or caregiver available?
A: Yes. There are many reasons why it is best to bring a caregiver or family member. But if no one is available, you may still come to the clinic.
